- Description
- Nursing Direct are looking to appoint a Registered Nurse with extensive mental health or learning disability experience to join our team in Purley. - We are looking for a Nurse to support our everyday operation and offer us clinical guidance and advice. - Nursing Direct specialise in providing support to patients who have transitioned into the community to receive care (most often complex) that is funded by their local authority. - The role - Full-time hours between Monday and Friday
- Primarily an office based role, however we do require you to visit clients within the community to conduct clinical observations and assessments
- Support the team with clinical responsibilities and advise
 - About - Carry out Individual care plan reviews for rota and complex clients. (Complex review monthly, rota reviews quarterly).
- Update reviewed care plans.
- Carry out clinical care assessments on new complex clients under manager guidance.
- Carry out detailed risk assessments on new clients and existing clients as needed.
- Identify any training needs of Care Assistants and Nurses and report accordingly.
- Report any changes in a client to the Care Consultant.
- Assist the Registered Manager to monitor, supervise and mentor all Carers and Lead Nurses on the packages.
- Assist Leanne to mentor staff through their competencies and sign off as competent.
- Assist Leanne to monitor all Service Users satisfaction.
- Collect all client care records for the month when on site and send them back to the Office.
- Ensure the accuracy and completeness of all documentation held in Client’s home.
- Attend MDT and safeguarding meetings as needed.
- Ensure that all written reports and Medication Administration records (MAR) are up to date.
- Write up and review Care Plans
- Support internal staff with all clinical queries
- Deal with safeguardings and incident reports in line with CQC and company standards.
- Interview new staff members and assess them for work with prospective clients.
- Liase directly with Clinical Commissioning Groups (CCG’s) over a clinical concerns, budgets etc.
 - Salary
