Decision For On-Boarding Care Provision
For Rapid Response Cases
Decisions for referrals can be made within 3 hours of receiving all the requested information to inform the care provision.
Extra - Package of Care Provision
Decision for referral can be made within 2 working days to ensure that there is relevant and adequate skill mix.
Respite / Short Break Packages / PBS Implementation / Adjustments to Service Provision
Decision for referral can be made on a rapid response basis, ideally within 3 hours - 2 working days.
COVID-19 Response Cases
Decisions for referrals can be made within 3 hours of receiving all the requested information to inform the care provision.
Nursing Direct will email the referrer with a decision to accept or decline to on-board care services. Where the decision is to decline, recommendations will be sent to you with rationale for the decision.
Referral Process to Nursing Direct
- Telephone consultation with Nursing Direct Clinicians/ staff where an on-boarding form will be completed.
- Nursing Direct will email the referrer a referral form and a list of documents to help in developing an appropriate proposal for a support package.
- For Referrer to complete Referral paperwork as requested by Nursing Direct and attach a signed/ evidence of agreed consent from service user/ someone with parental responsibility for those under age of 16, or any relevant document that support decisions to be made on behalf of the service user to reduce data breaches.
- Nursing Direct will email the referrer with a Care Services Agreement (CSA), which needs to be signed by the person responsible for Commissioning the care provision. Nursing Direct will accept an email confirmation of the CSA for up to 1 week from on-boarding services.
- For Rapid Response Referrals- Nursing Direct can be invited to a Professional meeting for consultation and a decision for on-boarding can be agreed after meeting provided there is valid consent in accordance to Nursing direct Policy. The costing for the service, can be agreed for a fixed period of time, and an email will be sent by Nursing Direct to the referrer for approval. Care can commence with the view of the CSA being completed and approved by the end next working day.
Referral Criteria
- Complex discharge planning and transitions back to the community service from hospitals, and other places of safety.
- Complex safety management protocols for service users who are at risk.
- Complex crisis prevention and management protocols for Behaviour that challenges.
- Emergency risk assessment packages- for the service users that may require supportive packages at home from recommendations from social care and mental health services.
- Adjustments to service provision, home environment support for service users at risk to themselves.
- Admission avoidance for service users at risk of admission without robust and consistent support packages.
- Interim support for transition and discharge back to the community.
- PBS informed care,- supporting implementation of a PBS care plan and support the review of PBS, Functional Behavioural assessment, mental health formulation and social care plans.
- COVID-19 Response to collaborative working with local networks which informs any ongoing assessments, treatment and development of personalised efficient enhanced community care.
- Support the progress of service through crisis recovery and reengagement back into education and community access.
- Comprehensive Care support Needs for failed homecare packages or placements.
- Support and training packages for teams for staff in placements as part of transition or implementation to prevent placement breakdown and risk of admission.
