To start the process, a Nursing Direct assessor will arrange a visit with you and your family to identify the services and support you will need before preparing a comprehensive care plan.

We work with a large number of CCG’s as well as other healthcare professionals to ensure that the quality of care that you get is consistent and constantly reviewed. It is important to us that you have a quick route back to speak to us when needed and will provide you with a dedicated liaison that will be familiar with your requirements.
