They are going to try and review any training that the staff have, and they are also going to try and find out what went wrong as well. This is especially interesting for those who work in homecare or even in private nursing. The investigation is going to explore how any clinical information is being dealt with and they are also going to be talking to parents and even carers. The HSIB have stated that they are going to try and consider any systemic or even environmental influences that are going to come into consideration and they are also talking about contextual factors. These are all going to influence how effective and on-track the trigger-tools are. This is interesting to say the least given the history of the service, but it is safe to say that more needs to be done in order to stop any more infants from dying needlessly just because of poor communication or lack of understanding.
Infant Death has Sparked a Huge Investigation
There has been a huge investigation done into the NHS to try and find out if they are recognising any seriously ill or even deteriorating infants. This has been put into place because a three-month old infant died soon after being sent home from a hospital. The HSIB have come out to say that they are the ones who have launched the investigation, and this is because they were notified about the death. They stated that the baby was admitted to hospital but was discharged soon after. The infant was brought back into hospital 4 hours later and then died from meningococcus. The HSIB have stated that they are working hard to try and understand the circumstances regarding this, and they are also trying to find out why the ill infant was not recognised at all.
